Hereinafter referred to as the animal, is being transferred to the legal adult and now adopting owner with the understanding that the adopter is taking possession of the animal to treat and be responsible for it as their own for the lifetime of the animal, which may be 10-20 years. *

If a dog, I will walk the dog on a leash and/or exercise the dog in a fenced yard, which must be provided unless waived by Epic. If a cat, the cat will kept as an indoor animal, not free to roam outdoors unattended. *

I will never let the animal run loose or roam, chain or tie up the animal, leave continuously in a yard/garage/ patio/balcony/pen, or leave the animal outdoors when no one is home. *

I will notify Epic immediately if the animal becomes lost or stolen and shall make every reasonable effort to help recover the animal. *

I understand Epic cannot guarantee the animals behavior now or in the future. I will make every effort to correct undesirable behaviors by taking the animal to a trainer/behavioral specialist at my expense. *

I will provide all required and/or needed medical expenses through a licensed veterinarian including, but not limited to: Rabies shots, DHLPPC, yearly fecal checks, heartworm test and provide prompt treatment for any illness or injury. *

If the animal is not neutered upon adoption, I agree to take the animal to the approved veterinary clinic by Epic on date scheduled and discussed with me by Epic. Further, I understand I am subject to fines & penalties described on the Epic Spay/Neuter contract if I miss the appointment and do not reschedule in advance with a Board Member of Epic. *

If a dog, I will give the dog heartworm preventive every month, all Epic long for the remaining life of the dog. I understand that I must retest for heartworms every year or if there is any break in giving tablets between months through a licensed veterinarian. *

I agree not to have cosmetic surgery performed on the animal, this includes but is not limited to: ear cropping; tail docking declawing, and; debarking. *

The animal will live inside my home and treated as a member of the family. I will provide shelter, food, fresh water and do my best to ensure the animals safety and well-being. *

I will pay the adoption fee of the animal in full and understand Epic may adopt-out the animal to another person if payment is not made. *

I understand that there is a 7 day trial period from adoption date of the animal. After the 7 day trial period, I understand that my donation/adoption fee is no longer refundable. *

I may notify Epic if the dog has health related problems during the 7 day trial period and seek Epic’s assistance for treatment at one of Epic’s approved veterinary clinics. I must receive written authorization from a Board Member of Epic before visit. If I choose to take the animal to a clinic not approved by Epic, Epic will not be responsible for expenses and will not reimburse services spent. *

I attest that any existing pets in the household owned are neutered, up-to-date on vaccinations and on heartworm preventatives. *

I am adopting the animal for myself and will not give, sell or abandon the animal for any reason. *

If I am unable to care for the animal, I understand that there is no return policy after the seven day trial period. Epic is not responsible to take back my family member at any time.

The animals medical and behavioral history has been discussed with me. I understand Epic cannot guarantee health, temperament, breed, age, or weight of the animal. *

Understand that Epic Animal Rescue has the right to inquire about any of the conditions above at any time after the adoption. I understand that failure to comply with any of the above provisions will result in forfeiture of the animal to Epic. *

Adopter agrees to indemnify and hold harmless, Epic Animal Rescue (HEREINAFTER REFERRED TO AS “Epic”), and their agents, employees, representatives, volunteers, or board members, for any damages and/or injuries to any person, property, or other animals as a direct or indirect result of the adoption and/or receipt of this animal. Further, Adopter indemnifies and releases Epic from any and all past, present and future liability, claims, rights, damages, debts, costs, causes of action, and demands of any kind whatsoever, whether based in tort, contract, or other theory of recovery, including, but not limited to any claim for personal injuries or loss of services or loss of consortium, which the Adopter or their heirs, administrators, or assigns, ever had, nor have, or may have in the future have, against Epic and for its employee or volunteers, arising by reason of or in any way related to or connected with, or that may be traced, either directly or indirectly to the adoption of the said animal. *
